Another useful classification of hypoglycaemic conditions is into those that are, and those that are not, associated with ketonaemia (plasma p-hydroxybutyrate levels greater than 600┬╡mol/l). Most types of hypoglycaemia, apart from those due to endogenous or exogenous hyperinsulinism, autoimmune hypoglycaemia, or IGF2-secreting tumours, are accompanied by mild-to-moderate ketonaemia. The main pathological causes of hypoglycaemia are shown in Fig. 20.9.
